gitsigns base=HEAD so ]c/[c reach staged-only hunks (previously silent no-op after git add without commit). Adds option+arrow line-move mappings for terminals that send them as modified escapes, alongside existing alt+j/k.

gitsigns base = HEAD so ]c/[c jump to hunks that exist only
between index and HEAD (git add without commit previously meant
no hunks and silent no-ops). Add <M-Up>/<M-Down> mappings in
normal and visual mode alongside <A-j>/<A-k>: macOS terminals
send option+arrow as modified escape sequences, so arrow-key
moving needs its own mappings.
